Eligibility for Services:

Choices Unlimited services individuals who participate in the SCL and Michelle P. wavier programs without regard to race, religion, disability, national origin or gender. Participates seeking services will be assessed fully to determine if they are candidates for Choices Unlimited services. Should Choices Unlimited be unable to meet the support needs of an individual, we will assist in identifying other available resources in the community.

Services Offered:

        button003_1Case Management- monitors all services and ensures individual programs are running as designed.

        button003_1AdultFoster Care / Residential Level 2- Alternative to staffed residence. Individuals live in a family oriented environment.

        button003_1CommunityLiving Support- One on one training for a variety of learning objectives for Michelle P. waiver recipients.

        button003_1Personal Assistance- One-on-One training for a variety of learning objectives for SCL waiver recipients.

        button003_1Respite- Allows some time for separation between caregiver and individual when needed.

        button003_1Personal Care- One-on-One assistance with eating, bathing and other activities of daily living.

        button003_1Community Access- Empowering individuals to participate in meaningful routines, events and community organizations.
